Care Coordinator / Care Assessor

Beluga Rox is recruiting an experienced Care Coordinator / Care Assessor on behalf of a multi-award-winning care and support provider in York.

Our client delivers a range of specialist complex care and support services to people of all ages. Since being established in 2017, the organisation has developed an excellent reputation among case managers, commissioners, healthcare professionals and private individuals.

Due to continued growth, we are looking for a compassionate, organised and motivated individual to join an established and supportive team. This dual role is an excellent opportunity for someone with domiciliary, community or complex care experience who enjoys coordinating services, completing assessments and ensuring clients receive high-quality, person-centred care.

The Role

Working closely with the Registered Manager, complex care nursing team and wider operations team, you will play a key role in the coordination and delivery of care services across York and the surrounding area.

You will be responsible for coordinating care packages and staff schedules, completing new client assessments and regular care reviews, developing person-centred care plans and supporting care teams to deliver safe, effective and compassionate care.

You will often be the first point of contact for clients, families, carers and healthcare professionals, providing a crucial link between everyone involved in a client’s care.

This is a varied and rewarding role where no two days are the same.

Key Responsibilities

  • Carry out new client assessments and regular care reviews.
  • Develop, update and maintain person-centred care plans and risk assessments.
  • Effectively schedule and coordinate care visits, ensuring continuity of care and appropriate staffing levels.
  • Manage the activities of the care workforce to ensure clients’ individual needs are met.
  • Maintain accurate and up-to-date records using digital care management systems.
  • Communicate effectively with clients, families, carers, healthcare professionals and other relevant parties.
  • Provide guidance, support and practical advice to members of the care team.
  • Work closely with the Registered Manager and wider operations team to ensure excellent service delivery.
  • Liaise with the complex care nursing team to coordinate joint assessments, client-specific training and staff competencies.
  • Respond promptly and professionally to changes in clients’ needs or care arrangements.
  • Promote the organisation’s services through professional face-to-face and telephone communication.
  • Complete general office administration duties as required.
  • Participate in the on-call rota and provide emergency support or care delivery when necessary.
  • Uphold the organisation’s values, policies and reputation as a high-quality provider of care and support services.

What We’re Looking For

  • NVQ Level 3 in Health and Social Care, or an equivalent qualification.
  • Previous experience delivering regulated personal care and support.
  • Experience within domiciliary, community or complex care.
  • Previous care coordination, assessment or care planning experience.
  • Experience working in a busy, fast-paced care environment.
  • Strong organisational skills and the ability to manage changing priorities.
  • Excellent verbal and written communication skills.
  • A compassionate, empathetic and person-centred approach.
  • Confidence communicating with clients, families, care staff and healthcare professionals.
  • Strong problem-solving skills and the ability to remain calm under pressure.
  • The ability to work independently while contributing positively to a wider team.
  • Good IT skills, including confidence using digital care systems, Microsoft Word and Excel.
  • A full UK driving licence and access to your own vehicle, as travel will be required to meet the needs of the service.
